作者叶文(2019)在《自体动静脉内瘘流出道静脉狭窄组织中非编码RNA的表达谱及生物信息学分析》一文中研究指出:目的:通过高通量测序(High-throughput sequencing)检测肾衰竭患者自体动静脉内瘘(autologous arteriovenous fistula,AVF)流出道静脉狭窄组织中长链非编码RNA(long noncoding RNA,lncRNA)的表达谱变化,探讨lncRNA与AVF流出道静脉狭窄的关系。方法:选择2017年2至6月在南昌大学第一附属医院肾内科诊断AVF失功患者再次手术的血管组织28例(实验组),纳入标准为内瘘使用时间≥3个月;内瘘透析时血流量≤200 ml/min;内瘘狭窄程度大于50%,排除有明显血栓形成者;上肢血管超声检查资料完整者。对照组选自同一机构尿毒症患者初次行动静脉内瘘术的血管组织75例,纳入标准为流出道血管内径≥1.6 mm,且无血管狭窄或其他血管疾病者。将103个样本采用Trizol法提取总RNA,分别选取实验组和对照组各4例,利用高通量测序分别检测其lncRNA及mRNA表达谱的差异,随后筛选出差异表达的lncRNA及mRNA,进行聚类分析,GO功能分析、KEGG pathway分析;并用实时荧光定量PCR(real-time RT-PCR,qRT-PCR)对RNA-seq结果进行验证;最后扩大样本量验证LINC01615在动静脉内瘘流出道静脉狭窄组织中高表达。结果:与对照组相比,表达变化≥2倍并且差异有统计学意义(P<0.05)的lncRNA或mRNA,认为是显著差异表达的lncRNA或mRNA。(1)lncRNA表达变化≥2倍的共247个,实验组表达上调≥2倍的有141个,表达下调≥2倍的有106个;(2)mRNA表达变化≥2倍的共1386个,实验组表达上调≥2倍的有874个,表达下调≥2倍的有512个;(3)GO功能分析提示显著差异表达的lncRNA主要参与了细胞的分化、凋亡;血管形态发生、血管发育、脉管系统发育等;(4)KEGG pathway分析提示差异表达的基因注释到PI3K-AKT信号通路是最多的,其显著性最高的是TGF-β信号通路。(5)在显著差异表达的ncRNA中,使用qRT-PCR技术对关键性的9个lncRNA及7个mRNA表达水平进行验证,其结果与测序一致;(6)与对照组相比,LINC01615在动静脉内瘘流出道静脉狭窄组织中高表达。结论:尿毒症血液透析患者动静脉内瘘流出道静脉组织存在显著差异性表达的lncRNA,这些lncRNA及某些信号通路可能在尿毒症患者AVF流出道静脉狭窄发生和发展中发挥重要作用。本研究可能为AVF流出道狭窄的发病机制的阐述奠定基础,并为AVF失功患者提供潜在的预防靶点。
