口腔白斑(oral leukoplakia,OLK)是口腔黏膜中最常见的癌前病变,组织学检查显示鳞状上皮单纯增生或/和异常增生者,在临床上有相当比例(3%～5%)的癌变率,平均癌变病程达8.2年。少数OLK恶变为鳞状细胞癌(oral squamous cell carcinoma,OSCC)。OSCC是口腔中最常见的恶性肿瘤,占口腔恶性肿瘤的80%以上。由于从癌前病变发展到癌是一个循序渐进的变化过程,故检测OLK的恶变潜能,探讨其与口腔鳞状细胞癌发生发展之间的关系受到学术界广泛关注。 近年来的研究成果显示血管生成的发生是肿瘤形成过程中的早期事件也是肿瘤发生过程中重要的一步。由癌前病变渐变为癌以及肿瘤形成后的发展、转移过程中,血管生成(angiogenesis,AS)起着重要作用。对肿瘤血管的生成和抑制的研究已经成为肿瘤学一个新热点。Endostatin是1997年O’Reilly和Folkman等从体外培养的鼠血管内皮细胞瘤(EOMA)的细胞培养液中提纯出一种新的蛋白质,相对分子质量为20KD;序列分析显示其为胶原蛋白XⅧC末端片段。Endostatin能抑制血管内皮细胞增殖和迁移、诱导内皮细胞凋亡进而抑制新生血管生长,以及肿瘤生长和转移。血管内皮生长因子(Vascular endothelial growth factor,VEGF)又称血管渗透性因子,是一种分子量为45KD的分泌性糖蛋白,可以促进内皮细胞增殖及血管新生,提高血管通透性,改变细胞外基质的成份和结构。目前普遍认为VEGF是已知的作用最强的促血管生成因子,在组织器官的发生、正常状态的维持以及多种肿瘤的发生、发展中都发挥着重要的作用。肿瘤
