论文摘要
目的分析剖宫产术后腹壁子宫内膜异位症的临床特点、治疗方法及预后。方法对经病理诊断确诊的51例剖宫产术后腹壁子宫内膜异位症患者临床资料进行回顾性分析并电话随访,结果经SPSS17.0软件进行统计分析。结果1.一般特征：51例患者平均年龄(32.08±3.46)岁。剖宫产结束至出现症状时间间隔平均(36.0±26.3)月,出现症状至就诊时间间隔平均(23.1±22.8)月。2.临床表现：78.4%(40/51)表现典型症状,即包块出现与月经周期相关的周期性疼痛并进行性增大。3.诊断及治疗：49例术前诊断为腹壁子宫内膜异位症,确诊率96.1%,手术切除病灶是治疗首选,15例术中切缘距病灶边缘1cm～2cm,36例术中切缘距病灶边缘>2cm。4.术后随访：随访的34例患者中20.6%(7/34)复出现瘢痕部位疼痛或包块,其中疼痛与月经周期关系明显者6例,7例中5例手术后没有追加药物治疗。结论腹壁子宫内膜异位症病灶切除术中切口边缘距病灶边缘2cm以上及术后追加药物治疗可以减少术后复发率。
