急性冠状动脉综合征患者血浆脑利钠肽水平变化及其影响因素

研究背景:急性冠状动脉综合征(acute coronary syndrome,ACS)是导致冠心病患者住院和死亡的主要原因,对其发病机制、诊断和防治的研究是近年来人们研究的热点。脑利钠肽(brain natriuretic peptide or B-type natriuretic peptide,BNP)是一种由心室合成、分泌的心源性激素,已被临床上广泛用于心力衰竭(heart failure,HF)的诊断和鉴别诊断,判断心力衰竭的程度、预后并指导治疗,被称为充血性心力衰竭的第一个潜在的“白细胞数”。最近有研究发现,ACS患者血浆BNP的水平也会增高,除心功能受损外,心肌缺血、坏死和炎症可能是使ACS患者血浆BNP水平升高的重要因素。目的:对ACS患者血浆BNP水平的变化及其影响因素进行分析,探讨BNP在ACS患者中检测的临床意义。研究方法:选择2006年9月至2006年12月在我院心血管内科住院平均年龄63.58±8.97(50~75)岁的患者100例,其中男性57例,女性43例。分为如下3组:(1)非冠心病对照组(NCHD组):20例,为无心肌缺血、无器质性心脏病证据和心功能正常者;(2)稳定性冠心病组(SCHD组):40例,包括稳定型心绞痛及陈旧性心肌梗死患者;(3)ACS组:40例,包括不稳定型心绞痛及急性心肌梗死患者。SCHD组及ACS组又根据左心室射血分数(LVEF)分为Ⅰ组(LVEF≥50%,n=20)和Ⅱ组(LVEF<50%,n=20)。使用美国博适Triage干式快速定量心肌梗死/心力衰竭诊断检测仪(美国Biosit公司产品)测定血浆BNP水平,同时分析BNP与其他传统危险因素之间的关系。结果:1.ACS组和SCHD组BNP水平(分别为509.87±80.84pg/ml和415.06±99.48pg/ml)均显著高于NCHD组(14.85±2.17pg/ml,P均<0.01);ACS组BNP水平略高于SCHD组,但未达到统计学差异(P>0.05)。2.在SCHD组及ACS组中心功能不佳(LVEF<50%)的Ⅱ组患者的BNP水平显著高于心功能较好(LVEF≥50%)的Ⅰ组患者(分别为797.4±158.77pg/ml对32.72±5.59pg/ml和892.55±104.70pg/ml对127.18±21.32pg/ml,P均<0.01)。SCHD和ACS两组中Ⅰ组和Ⅱ组患者的BNP水平均显著高于NCHD组患者(14.85±2.17pg/ml)(P均<0.01):ACSⅠ组患者的BNP水平显著高于SCHDⅠ组患者(P<0.01),ACSⅡ组患者的BNP水平高于SCHDⅡ组患者,但没有显著性差异(P>0.05)。3.直线相关分析显示BNP与白细胞计数(white blood cell count,WBC)、血清肌酸激酶(creatine kinase,CK)、高敏C反应蛋白(high sensitive C-reactive protein,hs-CRP)、左房收缩末期内径(left atrial diametric of systolic,LA)、左室舒张末期内径(left ventricular diametric of diastolic,LVD)和左室后壁厚度(poster left ventricular wall thickness of diastolic,LVPWT)呈显著正相关(r分别=0.287,0.210,0.511,0.577,0.679和0.217,P均<0.01),与LVEF呈显著负相关(r=-0.781,P<0.01)。4.多元线性逐步回归分析发现:hs-CRP、LVEF与BNP独立相关(回归系数B分别为0.222和-0.392,P分别=0.040和0.004)。结论:1.ACS患者血浆BNP水平明显升高,尤以心功能受损者升高更明显。2.hs-CRP和LVEF是BNP的主要影响因素。3.BNP水平的检测,不但是评价ACS患者心功能受损程度的敏感指标,而且可能是判断心肌缺血、坏死、炎症反应程度的有用指标。
