目的 观察缺血创面中Smad3与增殖细胞核抗原(PCNA)的表达特征,探讨缺血创面的难愈合性与Smad3之间的相关性,并研究Smad3与碱性成纤维细胞生长因子(bFGF)之间的关系。 方法 实验分为三组:单纯损伤组非缺血侧、单纯损伤组缺血侧及bFGF治疗组,后两组建立在兔耳缺血创面模型基础上。于伤后3、7、11、15、22天取创面组织标本,行HE染色观察上皮再生和肉芽组织形成的情况。行免疫组化染色,观测Smad3与PCNA在创面组织中的表达。 结果 单纯损伤组非缺血侧创面愈合最快,bFGF治疗组次之,单纯损伤组缺血侧创面愈合最慢。损伤后创面愈合过程中,Smad3、PCNA的表达呈现出一定的规律性。同一时间点上,单纯损伤组非缺血侧创面Smad3和PCNA阳性细胞率比bFGF治疗组和单纯损伤组缺血侧高。同一时间点上,bFGF治疗组创面Smad3表达阳性细胞率与单纯损伤组缺血侧相比差异无显著性,PCNA阳性细胞率与单纯损伤组缺血侧相比存在显著性差异。 结论 1、Smad3在愈合过程中参与了炎症反应、肉芽形成和胶原的合成过程。2、Smad3、PCNA在非缺血创面和缺血创面中的表达均呈现出一定的规律性。3、缺血创面的难愈合性可能与创面中Smad3、PCNA的表达时间延迟和表达阳性细胞数减少有关,合理调控其表达可能有助于有效促进慢性缺血创面的愈合。4、bFGF通过使缺血创面中进入增殖状态的细胞增加,从而发挥其促进愈合的作用。5、bFGF不能影响Smad3在缺血创面中的表达,Smad3作为一种细胞内信号转导分子可能不参与bFGF的下游细胞内信号传递。
