作者张亮(2019)在《益生菌复合固体饮料对便秘的缓解作用及其机制研究》一文中研究指出:便秘是一种常见的肠道疾病,主要表现为排便困难、粪便干硬和每周排便次数小于3次,发病率呈现出逐年上升的趋势。短期便秘影响人们的正常工作和生活,长期便秘甚至会导致肾病、心血管疾病的发生,因此对便秘的预防和治疗应引起人们的重视。便秘的产生受多种因素的影响,包括生活和饮食习惯的改变、心理状态的变化等。临床上常使用大黄、泻剂等治疗便秘,长期使用会带来很大的依赖性。研究表明,便秘患者肠道内肠道菌群产生紊乱,双歧杆菌和乳杆菌减少,致病菌和条件致病菌增加。这提示我们可以通过体外补充益生菌来调节肠道菌群,从而恢复正常的肠道运动,达到缓解便秘的目的。因此,本研究使用含有已在便秘动物模型上验证有效的Bifidobacterium bifidum CCFM 16与其他功能菌株复配,并添加低聚果糖、低聚木糖、菊粉等益生元成分,得到新型商业化益生菌复合固体饮料,验证商业化益生菌复合固体饮料对小鼠便秘的缓解作用及其相关机制,并探究其对人群便秘的缓解能力和机制,分析动物数据和人群数据的对应性。首先建立便秘小鼠动物模型,使用盐酸洛哌丁胺诱导小鼠便秘,随后灌胃不同剂量的商业化益生菌复合固体饮料,测定便秘小鼠的排便状态(排便粒数和粪便含水量)、小肠推进率和首粒黑便时间,发现中、高剂量的益生菌溶液能显著提高便秘小鼠的排便粒数和粪便含水量,并缩短首粒黑便时间和提高小肠推进率,与CCFM 16缓解便秘的效果相当,且商业化益生菌复合固体饮料在整体上更趋向于恢复便秘症状至正常值。研究商业化益生菌复合固体饮料缓解便秘的机制,发现中、高益生菌剂量组中小鼠粪便的乙酸、丁酸水平显著升高,乙酸和丁酸对肠道蠕动具有促进作用,与CCFM 16提高乙酸和丙酸的实验结果有所差异。商业化益生菌复合固体饮料的摄入显著改变了便秘小鼠肠道中紊乱的菌群结构,提高了Firmicutes、Actinobacteria的水平,降低了Bacteroidetes、Proteobacteria的水平;在属水平上提高了Bifidobacterium、Lactobacillus、Clostridium、Ruminococcus和Coprococcus相对丰度,使Blautia的相对丰度降低,其中Clostridium、Ruminococcus和Coprococcus与SCFAs(short-chain fatty acids,SCFAs)的产生有关。商业化益生菌复合固体饮料也能够调节血清中相关胃肠调节肽的水平,使其恢复至正常值,但是对5-羟色胺(5-HT)的浓度没有影响。其次为了探究商业化益生菌复合固体饮料对人群便秘的缓解能力及其机制,本实验进行了一项针对便秘患者的产品试食反馈实验,结果显示商业化益生菌复合固体饮料、益生元和安慰剂(麦芽糊精)缓解便秘的总有效率分别为80.0%、41.0%和33.0%。与便秘动物模型相似,通过对人群粪便SCFAs的测定,发现益生菌复合固体饮料显著提高了乙酸的含量。商业化益生菌复合固体饮料对便秘患者的肠道菌群产生了复杂的影响,各组患者在服用商业化益生菌复合固体饮料前后其肠道菌群的多样性和门水平上的相对丰度无明显差异,在属水平上外源性益生菌Bifidobacterium和Lactobacillus呈现出上升的趋势,同时Parabacteroides和Coprococcus也有所提升,Clostridium的相对丰度显著上升且Blautia的相对丰度显著下降,这与便秘小鼠模型的结果部分吻合。总之,商业化益生菌复合固体饮料在合理的剂量内具有良好的缓解便秘能力,与CCFM 16相比更倾向于恢复小鼠的正常排便功能,其缓解便秘的机制可能与乙酸含量的提高有关。商业化益生菌复合固体饮料一方面通过益生菌的发酵作用直接提高乙酸含量,另一方面通过乳酸和氨基酸等其他代谢产物促进与产SCFAs相关的Clostridium和Coprococcus的生长繁殖,进而引起SCFAs浓度的变化,刺激肠道蠕动,从而恢复肠道的正常转运。
