随着我国体育运动事业的不断发展,运动创伤也呈逐年上升趋势,其中前交叉韧带(Anterior cruciate ligament ACL)损伤最常见,损伤后可以产生明显的膝关节不稳,严重影响膝关节功能,继而发生膝关节退变与骨关节病的发生.因此为了更好的指导临床治疗,本文将简述前交叉韧带损伤,并介绍ACL重建手术中及手术后相关临床问题的探讨,其重点提高韧带的稳定性、促进移植物再血管化、选择合适的重建材料、关节镜下微创手术以及术后早期康复.本文通过对吉林大学中日联谊医院骨外科自2009年3月至2011年3月ACL损伤间收治共42例,临床资料完整的42例ACL损伤病例进行随访及回顾性研究,综合分析影响ACL损伤治疗的各种因素,为如何科学地选择治疗方法,改善病人预后,获得最大的功能提供客观的科学依据。本研究可以得出以下结论：通过收集并整理了2009年3月至2011年3月间吉林大学中日联谊医院骨外科治疗的42例临床资料完整42例ACL损伤病例,对患者出院时治疗结果及随访结果进行回顾性分析,术前、术后的膝关节稳定性和Lysholm评分进行对比,经统计学分析得出结论,无论是自体组、异体组、还是人工组,通过2年以上的随访,80%的患者通过术后膝关节稳定性评价Lachman试验以及轴移试验,自体组(胭绳肌肌腱)具有显著的优越性,但在关节功能评分方面短期内三组没有明显差异。总之,前交叉韧带重建手术术式不断改进,重建理念不断更新与进步,术后效果稳步的提高,隧道位置、移植物的选择、固定方式、术后康复是影响术后效果的主要因素,还需多中心、大样本、对比性的临床研究和更长期、更广泛的随访研究。
