论文摘要
1、目的：（1）发现与自发性静脉血栓栓塞症发生相关的实验室检测指标及其早期诊断价值，寻找自发性静脉血栓栓塞症的早期特异性诊断指标。（2）筛查自发性静脉血栓栓塞症的遗传易感基因，寻找中国人群相关的静脉血栓栓塞症的遗传易感基因。2、方法：（1）对连续收集的我院62例既往无血栓病史的首发的自发性静脉血栓栓塞症的住院患者（以及62例未发生静脉血栓性疾病的住院患者做为对照组）进行实验室相关指标，包括：血浆D-dimer含量、纤维蛋白原含量、白细胞计数、血小板计数和ABO血型的检测,并对这些实验室相关指标与自发性静脉血栓栓塞症的相关性分别进行单因素和多因素的统计学分析，寻找自发性静脉血栓栓塞症的早期特异性诊断指标。（2）记录连续收集的我院150例静脉血栓栓塞症的住院患者（以及112例未发生静脉血栓性疾病的住院患者做为对照组）的一般临床资料及获得性危险因素；并应用基于Sequenom MassARRAY DNA质谱阵列基因分析系统的iPLEX Gold snp基因型分析技术对静脉血栓栓塞症相关的20SNPs进行检测，并对这些遗传易感基因与静脉血栓栓塞症的相关性分别进行单因素和多因素的统计学分析，寻找中国人群静脉血栓栓塞症的独立遗传易感基因。筛查出150例血栓组中含有63例自发性静脉血栓栓塞症患者，选择年龄、性别匹配的63例对照组患者，应用单因素和多因素分析筛查这20SNPs中与自发性静脉血栓栓塞症相关的独立遗传易感基因。3、结果：（1）单因素统计分析发现，血浆D-dimer含量、纤维蛋白原含量、白细胞计数和ABO血型在血栓组与对照组间存在显著的差异（P<0.05）,而血小板计数在两组间差异无统计学意义（P>0.05）。对血浆D-dimer含量、纤维蛋白原含量、白细胞计数、血小板计数和ABO血型与自发性静脉血栓栓塞症之间的相关性进行Logistic回归分析发现，血浆D-dimer和纤维蛋白原含量为自发性静脉血栓栓塞症的实验室独立危险因素，其OR值分别为D-dimer332.79（95%CI:29.88～3706.49,P=0.00）和纤维蛋白原含量19.01（95%CI:2.37～152.18,P=0.006）。将纤维蛋白原>4g/L为界进行分层后，血栓组与对照组间存在显著性差异（P=0.00），诊断的灵敏度为89.29%，特异度为56.63%，阳性预测值为40.98%，阴性预测值为94%。将血浆D-dimer>0.5mg/L为界进行分层后，血栓组与对照组间存在显著性差异，诊断的灵敏度为92.31%，特异度为96.61%，阳性预测值为96.77%，阴性预测值为91.94%。（2）将150例静脉血栓栓塞症患者和112例对照组经单因素分析有统计学差异（P<0.2）的获得性和遗传易感基因进行Logistic回归分析证实：年龄是静脉血栓栓塞症的独立危险因素，其OR值1.027（95%CI：1.008～1.047），rs1805087、 rs3756008、 rs2036914、 rs2066865和rs3087505是中国人群静脉血栓栓塞症的独立相关因素，rs1805087-G的OR值0.281（95%CI：0.105～0.753），rs3756008-G的OR值0.28（95%CI：0.12～0.653）,rs2036914-T的OR值0.392（95%CI：0.180～0.853），rs2066865-C的OR值0.381（95%CI：0.163～0.892），rs3087505-T的OR值5.248（95%CI：1.717～16.043）。将63例自发性静脉血栓栓塞症患者和63例对照组患者经单因素分析有统计学差异（P<0.2）的遗传易感基因进行Logistic回归分析亦证实：rs3087505是中国人群自发性静脉血栓栓塞症的独立危险因素，rs3087505-T的OR值3.760（95%CI1.009-14.006）。4、结论：（1）血浆D-dimer和纤维蛋白原含量是自发性静脉血栓栓塞症的实验室独立危险因素，D-dimer>0.5mg/L对自发性静脉血栓具有早期特异性诊断价值,通过结合临床表现可能性可进一步提高自发性静脉血栓栓塞症的早期诊断率。（2）MS、F11、FGG、KLKB1可能是中国人群静脉血栓栓塞症的遗传易感基因；KLKB1可能是中国人群自发性静脉血栓栓塞症的遗传易感基因。
