异基因造血干细胞移植后出血性膀胱炎的危险因素分析

论文摘要

目的:探讨异基因造血干细胞移植（Allo-HSCT）后并发出血性膀胱炎（hemorrhagic cystitis HC）的危险因素。方法:对2003年6月至2007年9月在新疆医科大学第一附属医院血液科行Allo-HSCT治疗的80例患者的临床资料进行回顾性分析。以预处理之日为观察起点,随访至移植后至少+100日。选择6个临床参数:性别、年龄、移植病种、ABO血型匹配、人类白细胞抗原（HLA）相合程度、急性移植物抗宿主病（aGVHD）的发生做单因素分析。多因素分析中选择有意义的单因素及临床认为可能的影响因素,应用logistic回归分析。结果:18例患者（22.5%）发生HC,均为迟发性（LOHC）,其中Ⅰ度8例（10%）,Ⅱ度5例（6.25%）,Ⅲ度4例（5%）,Ⅳ度1例;发病时间为+8～+83d（中位时间+28.5d）,持续时间3～62d（中位持续时间9.5d）。单因素分析发现年龄和HLA相合程度与HC有关（P值均<0.05）;多因素分析发现仅HLA相合程度与HC有统计学关联。结论:HC是Allo-HSCT后的重要并发症,HLA配型2-3个位点不相合移植是其发生的危险因素。
