组织多普勒超声显像对严重器质性二尖瓣返流引起左心室功能不全及扩张的评价和预测

背景:由二尖瓣脱垂及连枷样二尖瓣引起的慢性严重性二尖瓣返流会造成左心室的功能衰竭和扩张。然而这一进展过程及预测因子并没有被解释透彻,特别是目前尚无运用组织多普勒方法(TDI)进行研究的报道。本研究拟用TDI对这一过程加以探讨,并推测TDI对于左心室的代偿失调有预测作用。方法:连续入选中等程度以上(>Ⅲ级)的器质性二尖瓣返流病人153例(54±17岁,52%男性),病人的左心室射血分数都在正常范围(LVEF≥55%)。所有病人都进行前后两次常规超声心动图检查及脉冲TDI检查(基线、随访),两次检查相隔时间为650±362天。所有病人按随访检查左心室收缩末期容积是否超过基线的20%分为左心室显著扩张组和左心室未显著扩张组。结果:病人随访检查经体表面积纠正后的左心室收缩末期容积(17±5 to20±6 ml/m2,P<0.001)、舒张末期容积(49±12 to 55±13 ml/m2,P<0.001),左心房容积(35±14 to 42±17 ml/m2,P<0.001)都较基线检查有显著增加,二尖瓣返流程度和TDI的E/E’指标也有显著增加。TDI的左心室二尖瓣瓣环(侧壁、室间隔)的早期舒张速度(E’)却有显著减低,两次检查LVEF、左心房泵血分数(LAEF)、TDI二尖瓣瓣环收缩速度(S’)及舒张晚期速度(A’)基本相同。40例病人左心室有显著性扩张(26%),较左心室未显著扩张组,其基线的TDI S’及A’值更低,LAEF更低,其值与随后发生的左心室扩张程度有显著相关性。多变量分析证实基线A’是左心室扩张与否的独立预测因子(P<0.001),Receiver-Operator-Characteristic曲线下面积为0.78(95%CI 0.70-0.86)。A’<11.0cm/s预测随后左心室显著扩张的特异性为90%,敏感性为64%。结论:慢性严重的器质性二尖瓣返流,返流量不断增加的同时,左心室和左心房的重塑也在不断进展。TDI检查A’降低(左心房泵血功能降低)可以预测随后左心室的扩张。
