目的 探讨原发性胆汁性肝硬化（PBC）的临床、免疫学及病理学特点,以提高对本病的认识、诊断和治疗。 方法 分析78例原发性胆汁性肝硬化患者的一般资料、临床表现、免疫学及病理学等改变,同时对其中19例患者进行肝组织学检查。 结果 本组患者中女性70例,确诊时平均年龄为46岁。发病至确诊时间为1—480月。确诊时有20例患者无典型临床症状（25.6%）;有典型的皮肤瘙痒36例（46.2%）、肝脏肿大25例（32.1%）、脾脏肿大26例（33.3%）、有皮肤、巩膜黄染者64例（占82.13%）、ALP和GGT在疾病早期即明显升高,AMA阳性者为69例,约占88.5%;M2阳性59例,占75.6%,19例行肝穿刺检查的确诊患者中血清学检测均为阳性,UDCA治疗有效。 结论 我国PBC患者中也以女性为多,确诊时可无典型临床表现,如出现黄疸症状,预后差,进一步证实AMA-M2在PBC中的诊断作用,以及熊去氧胆酸在PBC中的治疗作用。
